The Patient Services Department at Cornerstone Family Healthcare improves access to health care and assures that patients can access health care services regardless of their insurance status.

Certified Application Counselors help patients access medical services by providing health insurance application and enrollment assistance via the NY State of Health Marketplace.

Insurance options include:

  • Child Health Plus
  • Essential Plan
  • Qualified Health Plan (QHP)
  • Medicaid/Prenatal Medicaid/Emergency Medicaid

Additional assistance provided includes:

  • Sliding Fee Scale Discount Program
  • SNAP (Supplemental Nutrition Assistance Program (Food Stamp)
  • HEAP (Home Energy Assistance Program)

Sliding Fee Scale Discount Program
The Sliding Fee Discount Program assures that patients without health insurance coverage have access to all primary care services. Eligibility for the sliding fee scale discount is based upon poverty guidelines established by the federal government and are determined by family size and income.

Privately Insured Patients
Patients with high-deductible health plans that require out-of-pocket expenses in excess of $2,500 annually will be extended the opportunity to apply for a sliding fee scale discount. Determination is based on documentation and verification requirements. If eligible, Cornerstone will apply the sliding fee discount by charging a fee no greater than what would be charged to a patient that has no public or private insurance in a defined income category.

Sliding Fee Scale Qualifications
Patients interested in the Sliding Fee Scale Program should make an appointment with the Patient Services Department at 845-926-3636.

To apply for sliding fee services and alternate payment arrangements you will need to provide proof of household income. We will require a current valid photo I.D. and any one of the following items:

  • All pay stubs from the past 30 days
  • Letter from your employer or a declaration of income
  • Unemployment benefit letter

Please allow at least one hour prior to your medical appointment to apply for the Sliding Fee Scale Program.
