Cornerstone Family Healthcare is an accredited Patient Centered Medical Home Level III based on 2014 standards in addition to being accredited by the Joint Commission earning their Gold Seal of approval.


The PCMH is best described as a model or philosophy of primary care that is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety.
It is a place where patients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff.

Patients are able to access services around their schedule, with shorter waiting times, “after hours” care (our hours), 24/7 electronic or telephone access, and strong communication through our patient portal which allows you to schedule appointments, ask a question of your physician, view test results, immunization records or receive important information about your visit. In addition all of our medical departments maintain same-day access to our providers. Please do not hesitate to contact us and request a same-day appointment.

Copies of your Health Records are available upon request. To receive a copy of your Health Record, please call our center at (845) 563-8000. A signed consent form allowing us to provide you or your specialist with a copy of your health records will be required.

Farm Stand
On a monthly basis, Cornerstone Family Healthcare operates a Free Farm Stand at the Newburgh Armory. For more information please contact our Health Education Department by calling (845) 563-8043.
Fit for Life
Nutrition and physical exercise are important in keeping you healthy. Fit for Life offers nutritional assistance and support, along with exercise and physical activities. For more information, contact our Health Department by calling (845) 563-8043.


    Behavioral Health Services Available


Cornerstone Family Healthcare offers behavioral health services to patients in need. All patients may access these services, which is located at our Kaplan Family Pavilion site in Newburgh. Behavioral health is the scientific study of the emotions, behaviors and biology relating to a person’s mental well-being, their ability to function in everyday life and their concept of self.

“Behavioral health” is the preferred term to “Mental Health”. A person struggling with his or her behavioral health may face stress, depression, anxiety, relationship problems, grief, addiction, ADHD or learning disabilities, mood disorders, or other psychological concerns.

Our Behavioral Health services include; counselors, therapists, life coaches, psychologists, nurse practitioners or physicians who can help manage behavioral health concerns with treatments such as therapy, counseling, or medication.


    Ability to Pay


Cornerstone Family Healthcare provides essential medical services, regardless of a patient’s ability to pay. If you are uninsured or have a high insurance deductible, a sliding scale fee discount is offered. Discounts are based on family/household size and annual income, as compared to the most current Federal Poverty Guidelines.
