The Health Literacy and Community Education Program at Cornerstone Family Healthcare is dedicated to empowering patients and community members to increase control over, and to improve physical, social and mental health by providing accurate, and accessible information about health and its determinants. Cornerstone collaborates with other health and human service organizations to foster a sense of shared community and to best provide the diverse learning opportunities that support the inherent ability of all individuals to create health and wellness for themselves, their families, and society.
