First Time Patients

Welcome to our practice and thank you for choosing Cornerstone Family Healthcare to be your health care provider. We understand that your time is valuable, so in order to expedite your visit, below is a list of items you will be required to provide upon your arrival to the Health Center.

Please bring the following items to your first visit:


  • Picture identification i.e., passport, driver’s license, green card (For children, please bring their birth certificate)
  • Insurance card(s)
  • Co-payments (We accept most major credit cards and checks for payment)


Most uninsured patients with appropriate documentation can be enrolled in our Sliding Fee Scale Program (see sliding fee scale section below) and will be eligible to pay a lower fee per visit.


Income verification will be required for all uninsured patients. If income verification is not provided, please be prepared to pay $173.00 for the visit.


If your health care provider orders lab work, medical treatments, immunizations, PPD’s, EKG, etc., an additional cost will be incurred and you will be required to pay at the time these services are rendered to you.


Sliding Fee Scale

Cornerstone Family Healthcare provides essential medical services regardless of a patient’s ability to pay. If you are uninsured or have a high insurance deductible, a sliding fee discount is offered based upon family/household size and annual income, and calculated according to the most current Federal Poverty Guidelines.

For more information, please ask your Patient Services Representative, see our Enabling Services Department or call our Health Center’s Call Center at (845) 563-8000. We recommend that you allow at least one hour prior to your appointment to apply for these services.

Documentation of household income for yourself and any individual residing with you is require when applying for sliding fee discount. Any of the following documentation is acceptable:

  • Proof of Income
    • last 4 pay stubs if paid weekly
    • last 2 pay stubs if paid bi-weekly
  • W2 tax statements
  • Letter of support
  • Unemployment benefit letter


Our Patient Services Representatives can assist you with qualifying your child for health insurance if he/she is uninsured.


Also, multiple managed care plans have representatives on-site you may speak with about low or no-cost health insurance.

There are many locally-based insurance programs, most of which are provided by unions or small employers that cover some of our services. If you are a participating member of one of these carriers please be sure to check with the carrier to ensure that your carrier will cover the services provided by Cornerstone Family Healthcare.

Under a new state law, effective April 1, 2015, healthcare professionals in New York State will be required to disclose to patients information pertaining to out-of-network providers whose services might not be covered by their insurance, allowing patients to make more informed decisions and avoid any unexpected or surprise billing. Furthermore, patients will now have the right to assign insurance benefits to non-participating providers and be charged only for applicable copays, coinsurance, or deductibles that would have been owed to a participating provider; physicians will have the right to bring disputed payments of bills for emergency services and surprise bills to independent dispute resolution.
